This survey is not a family survey but is for every individual 18 years old & older. If you have children away at school who have not registered their email address with us then send them the generic link or direct them to this website. If you are not registered with St. Matthew Parish but wish to participate, please use the Generic Link below.
The survey will be emailed to you by DiscipleMakerIndex@measuring-success.com. If you don’t receive an email, please check your “spam/junk folder”.

- Those with submitted email addresses will receive a unique email link from DiscipleMakerIndex@measuring-success.com. If a couple/family shares an email address, one person can use the emailed link and others should use the generic link.
- Please help us participate by completing a 10-15 minute survey online.
- Available from January 18th through Ash Wednesday, February 10th
- All responses will be kept confidential.
- The parish will only receive information about the community as a whole.
